What is the Severity Risk Index (SRI)?
The Severity Risk Index (SRI) is a compliance-based score derived from official Texas state inspection data.
Lower scores indicate fewer and/or less severe deficiencies over time. Higher scores indicate more frequent and/or more severe deficiencies,
and may also reflect enforcement status when corrective or adverse actions are present.

The Hall of Fame honors a select group of licensed childcare providers in San Antonio who have demonstrated strong,
consistent compliance over time. To qualify, centers must maintain an SRI of 0.40 or lower and have undergone at least 10 state inspections.

Medal tiers recognize long-term excellence within 10+ inspection history.
Needs Improvement Tier: Centers with SRI ≥ 1.50 fall into the Needs Improvement tier.
This designation is not based on parent reviews—only official inspection records.
Enforcement Actions: Corrective and adverse actions are issued by the Texas Health and Human Services Commission (HHSC),
under its Child Care Regulation (CCR) division. This enforcement flag may apply to centers at any SRI level.
